There is growing evidence to support prehospital, on-scene, immersive cooling for all exertional heat strokes..."Cool First, Transport Second". This has been best practice for sports medicine sideline care as well as in the military and other events such as marathons. However, EMS has been slow to adopt this method outside of sporting/mass gathering events. There is little to no evidence on how EMS should cool classical (non-exertional) heat stroke in the field. We describe the first retrospective case series of EMS immersive cooling of classical heat stroke patients using real-time rectal thermometry.
