Abstract: Aim: The diagnosis of VRFs in endodontically treated teeth is a challenge. The study aimed to evaluate and compare the accuracy of digital periapical radiography (DPR) and cone-beam computed tomography (CBCT) for detecting vertical root fractures in single-rooted endodontically treated teeth restored with either fiber resin posts or stainless-steel posts. Methods: Sixty single-rooted samples were decoronated 2mm above the CEJ. All samples were prepared using a rotary system till the working length and obturated with gutta-percha. Samples were then divided into two groups based on the post type they would receive. Fracture induction was done in half of the samples with the help of a chisel and hammer. Samples were then mounted on the mandible and subjected to CBCT and DPR scans. Image interpretation was done by three observers and readings were given based on a five-point scale. Statistical Analysis was done based on these readings using SPSS version 21.0 and Epi-info version 3.0. Results: The Inter and Intra examiner reliability of the examiners was found to be higher for CBCT. When post material was considered individually, examiner reliability was found maximum for the CBCT for both Posts. All fractures were identified correctly with maximum sensitivity values for fiber post in CBCT. Conclusion: CBCT was found to be more accurate in comparison to DPR for both the type of posts i.e., fiber resin post and stainless-steel posts. Overall CBCT is a better diagnostic modality in detecting all kinds of fracture situations i.e., complete/incomplete, and Bucco-lingual / Mesio-distal fractures.
