Abstract: Photobiomodulation (PBM) has been used for decades in medicine and dentistry showing promising but mixed results with its healing and analgesia capabilities. However, an accepted protocol has not yet been developed for endodontic use. The aim of this study was to investigate the anti-inflammatory and healing properties of PBM in vitro and in vivo. Human periodontal ligament (PDL) cells were challenged with lipopolysaccharide and exposed to 5J/cm2, 10J/cm2 and 20Jcm2 using a 660 nm diode laser. Cell viability was investigated using a CCK-8 assay. Messenger RNA (mRNA) expression of CALCA, BDNF, IL1A, IL-6, TIMP-1, and TNFα was examined via quantitative reverse transcription polymerase chain reaction. Eight patients diagnosed with symptomatic irreversible pulpitis and symptomatic apical periodontitis (1st and 2nd molars only) were selected to participate in an initial randomized clinical trial to evaluate the efficacy of PBM in reducing pain symptoms after conventional endodontic treatment. Cells exposed to 10J/cm2 of diode laser showed the best cell proliferation rates after 24 hours (p < 0.05; ANOVA). Cells initially challenged with LPS followed by 10J/cm2 laser treatment showed decreased CALCA, BDNF, IL1A, IL-6 and TNFα mRNA expression when compared with cells exposed to LPS without laser treatment (p < 0.05; ANOVA). Patients exposed to 10J/cm2 laser showed improvement in their symptomatology six hours after treatment completion when compared to the placebo group (p < 0.05; Chi-square and F distributions tests). Preliminary data supports the use of PBM as a possible effective means to reduce postoperative pain in patients who have received conventional root canal therapy.
