Saving teeth with peri-radicular lesions of endodontic origin due to failure of the previous therapy or peculiar pathological conditions is more predictable and less invasive with modern surgical Micro endodontics, with a reported success rate of more than 95%. The careful case assessment at the patient, site and tooth levels may guide decision making process to a less stressful procedure, a more favorable cost-effectiveness and outcome and with an improvement of the quality of life of the patients. During each phase of the procedure the clinician may face unexpected difficulties that require a rapid solution. The lecture will describe the principles of decision-making process (surgical vs non-surgical option, complete retreatment vs direct surgical approach, free-hand vs guided, the management of soft and hard tissues, the prognosis of endo-periodontal lesions). The impact of pre-surgical 3D digital planning will be also discussed. In the lecture video clinical cases and useful tips and tricks will be illustrated under the light of the existing evidence.
