(PR77) Influence of Restoration in the Healing of Periapical Pathology After Root Canal Treatment at Rutgers Endodontic Department

Abstract: Endodontic treatments are predictable and highly successful procedures. Several factors have been associated with increased success, such as pre-operative absence of a periapical radiolucency and adequate post-operative coronal restoration. The purpose of this study is to explore the association between delayed crown placement and radiographic quality of the root canal therapy on the healing of periapical pathology after root canal treatment. Study data was collected from Rutgers School of Dental Medicine electronic health records. 270 records of patients receiving initial non-surgical endodontic treatment (RCT) in the post-graduate endodontics clinic from 2016-2021 met inclusion criteria. Records were reviewed to determine elapsed time and type of permanent restoration placed after RCT. Radiographic acceptability of crown margins, root canal quality and periapical index (PAI) were also recorded. The average amount of time between the root canal and full coverage restoration was 8.31 months. 43.3% of recall patients obtained a full coverage restoration and 20.9% obtained a direct restoration. 90.1% of the full coverage crowns were acceptable. The overall success rate of RCT was 85.82%. The success rate of root canal with full coverage crown was 90.5%. The success rate of root canal treatment without full coverage crowns was 82.2%. The odds of success at the recall appointment were almost 3 times larger when the tooth was restored with a permanent restoration [OR = 2.90, 95% CI (1.43,5.84)]. Successful results were observed more frequently in teeth with permanent restorations than in teeth with temporary fillings.
