(PR65) The Effect of Implant Removal on the Survival of the Surrounding Teeth

Abstract: Given the increasing number of implants being placed, more implant complications and failures are expected. Several studies investigated implant failure due to peri-implantitis, poor osseointegration, and implant fracture. Bone quality and quantity have also been shown to play a significant factor in implant success and failure rates. Bone Type I and II are associated with higher implant success compared with Type III and IV.

Purpose: This retrospective study evaluated the effect of implant removal on the survival of the adjacent natural teeth and the cause of implant failure. It was hypothesized that there would be a statistically significant difference in the survival of natural teeth adjacent to the implant removal site. Materials and

Methods: 160 charts were reviewed for patients who received implant removal. Survival of teeth adjacent to a removed implant site and contralateral teeth were recorded. Outcomes were analyzed using Chi-square test(p <.05).

Results: 47 implant sites with a 22-month follow-up were reviewed. 6 teeth (12.7%) adjacent to removed implants were lost, compared to 1(2.1%) adjacent to natural teeth (p=0.1109). Causes of implant failure were the following: 85.1% (40/47) peri-implantitis, 10.6% (5/47) no osseointegration, 4.3% (2/47) implant fracture. Implant failure based on bone type: Type I (0), Type II (25.5%), Type III (34%), Type IV (40.4%).

Conclusion: The survival of natural teeth adjacent to removed implants was not affected, however, clinically the teeth should be evaluated to ensure they are endodontically sound.
