(TC36) Retrofill Resorption: Can It Lead to the Relapse of Apical Periodontitis?

Abstract: Calcium silicate-based materials are favored in the field of endodontics for their biocompatible, anti-bacterial, and bioinductive properties. These materials have gained wide acceptance among clinicians, especially when used as a root-end filling material for their superior sealing ability. Despite the desirable characteristics of calcium silicate-based materials, the differences in the solubility among these materials shown in physicochemical in vitro studies may have clinical implications regarding the long-term stability of root-end filling materials. Among calcium silicate-based materials, EndoSequence® BC RRM™ (ESBC-RRM) (Brasseler USA, Savannah, GA) has been used competently as a root-end filling material for endodontic surgery. It is composed of tricalcium silicate, tantalum pentoxide, calcium phosphate monobasic, and zirconium oxide and has been shown to demonstrate excellent biological and mechanical characteristics that make it a suitable root-end filling material. However, few clinical studies have investigated the long-term dimensional stability of ESBC-RRM when it is used as a root-end filling material. The purpose of this table clinic is to present a clinical case with a long-term follow-up of six years that demonstrates the relapse of apical periodontitis due to the resorption of the combination of ESBC-RRM putty and paste when used as a root-end filling material. Possible factors associated with the material resorption will be discussed based on its physicochemical properties. With this knowledge, clinicians will be able to manage the possible sequelae of retrofill resorption and relapse of apical periodontitis.
