(PR48) Prevalence of Different Types of Periapical Lesions in a Philadelphia Patient Population: A Retrospective Study

Abstract:

Objective: Studies have been performed regarding the prevalence of various types of periapical lesions. However, the results vary depending on the patient population and demographics. The purpose of this study is to determine the prevalence of different types of periapical lesions from the biopsy sample and pathology reports in the diverse and multi-ethnic Philadelphia patient population. Materials &

Methods: Data was gathered from 150 pathology reports from January 2016 to August 2021 at Temple University Kornberg School of Dentistry Graduate Endodontics Clinic and Endodontic Specialists, PC, a private endodontic practice in Greater Philadelphia. The data recorded includes tooth position, patient age, gender, pre-operative diagnosis, periapical radiolucency lesion dimensions, and pathology report diagnoses. Reports were categorized as follows: 1) granulomas 2) abscesses 3) radicular cysts 4) other.

Results: Among the 147 pathology samples following apicoectomy: 59.8% of lesions were granulomas, 16.2% abscesses, 22.3% radicular cysts, and 1.7% other (fibrous connective tissue and odontogenic keratocyst). Some samples presented as combination lesions: 15.1% granuloma and abscess combination and 3.9% as radicular cyst and abscess combination. There were 3 samples obtained without apicoectomy due to non-odontogenic lesion, 2 were odontogenic keratocyst and one was lateral periodontal cyst.

Conclusions: Granulomas have the highest prevalence among periapical lesions, followed by cysts and then abscesses. Non-odontogenic lesions are rare but are essential to biopsy and diagnose in order to provide better care for patients.
