(PR20) Photospectrometric Analysis of Post-obturation Silver Diamine Fluoride Application on Perceptible Shade Changes Prior to Restoration

Abstract: Silver Diamine Fluoride (SDF) is an efficacious antimicrobial agent known for arresting dental caries but is not utilized in endodontics. One advantage of SDF over traditional medicaments is that dissociated silver ions may provide longer lasting antimicrobial effects by penetrating dentinal tubules up to 40µm, however there is concern whether SDF may cause staining. The primary study objective was to evaluate whether SDF produces a perceptible shade change when applied intracoronally following obturation. Extracted human posterior teeth were endodontically treated under magnification using rotary instrumentation and obturated using a warm vertical condensation technique. Teeth were randomly assigned to one of three treatments: SDF (n=19), 0.9% sodium chloride [negative control (n=7)], or class V composite restoration [positive control (n=3)]. Initial baseline and 24hr post-treatment shade readings were collected using a spectrophotometer to generate color values in the CIE L*a*b* system in a standardized location 3mm coronal to the buccal CEJ. The average 24hr post-treatment ΔE*ab was 14.1, 12.4, and 20.4 for the SDF, negative control, and positive control treatments, respectively. The averages between the SDF and negative control treatments were not statistically significant, while a significant difference was calculated with the positive control (Tukey pairwise comparison, one-way ANOVA, p=0.05). Although silver ion staining was readily observed in the treated access cavity, it was insufficient to cause a perceptible shade change in the anatomical crown. Consequently, SDF warrants further investigation regarding applications to endodontics.
