Abstract: Objective: The aims of the Patient-Centered Dental Home project are to 1) develop a PCDH definition and quality measurement framework that broadens the scope of prior dental home definitions to include populations across the lifespan in a broad range of settings, 2) provide a quality measurement framework to align PCDH characteristics with quality indicators to facilitate quality assessment and improvement, and 3) promote opportunities for medical-dental integration through alignment with existing PCMH models.
Methods: Over the past four years, the project team has administered a series of Delphi surveys to a broad-based, 50+ member National Advisory Committee to identify the essential characteristics, components, and measure concepts within a PCDH definition and quality framework. Members have been asked to rate, on a scale of 1-9, how essential each element is to a PCDH model.
Results: A PCDH was defined as a model of care that is accessible, comprehensive, continuous, coordinated, patient- and family-centered, and focused on quality and safety as an integrated part of a health home for people throughout the life span. A quality measurement framework consisting of six characteristics, 21 components, and 61 measure concepts was developed. A searchable dental quality measure clearinghouse was also developed as part of this work.
Conclusions: The PCDH model and framework aim to be useful to practices, organizations, and systems with an interest in dental quality measurement and improvement and in medical-dental integration. The context for PCDH development, methods and results of the PCDH framework and future directions will be presented.
