602 - Simulation to Promote Management of Client from Hospital to Home

The associate degree nursing student has limited clinical opportunities involving the process of transitioning the client from acute care to the home setting. A simulation using a standardized client was created to help the student develop a holistic, thorough, and individualized discharge plan of care. The student conducted a comprehensive assessment, utilized therapeutic communication strategies, and identified resources to promote client self-management. Through this process, the student gained a better understanding of all that is involved in the discharge process to ensure the client experiences optimal outcomes
