P1073 - Pancreatitis Quality of Life Comparison in Patients With Chronic Pancreatitis

Introduction: Acute pancreatitis is the most common gastrointestinal etiology for inpatient hospitalization with estimated associated healthcare costs of 2.6 billion dollars annually. Surveys of patients with chronic pancreatitis showed that patients cited depression (52%), negative social life (60%), and unemployment (63%) secondary to their disease, strongly suggesting that chronic pancreatitis is a debilitating condition. Further studies are needed to target interventions for factors associated for quality of life.

Methods: Retrospective review of medical records was conducted for patients with chronic pancreatitis who completed pancreatitis quality of life instrument (PANQOLI). PANQOLI is the first validated instrument designed specifically for evaluation of quality of life. Primary outcome measured was quality of life using the PANQOLI score. Secondary outcomes included demographic data, number of hospitalizations, medications including psychiatric drugs and opioid use, and smoking and alcohol history. Continuous variables were compared using the t test or Mann-Whitney U test. Categorical variables were compared using the Fisher exact test or Chi-square test. Multiple linear regression was used to determine independent predictors for PANQOLI score.

Results: 76 patients with chronic pancreatitis were included; 64.5% females with an average age of 49.2 ± 9.9 and 86.8% were white. The average PANQOLI score in our cohort was 55.7 ± 13.5. On univariate analysis, female gender, white race, diagnosis of depression, psychiatric medication usage and smoking was significantly associated with lower PANQOLI scores (Table 1). In the final adjusted linear regression model, patients with depression (β = -7.4; 95% CI, -13.8- -1.1; P= 0.022), patients on psychiatric medications (β = -6.8; 95% CI, -12.8 -0.8; P = 0.027), and smokers (β = -7.2; 95% CI, -13.6- -0.8; P = 0.028) predicted lower scores. Non-white patients had higher PANQOLI scores (β = 8.2; 95% CI, 0.3-16.2) compared to whites. Alcohol use was not associated with PANQOLI score (p=0.15).

Discussion: Worse quality of life in patients with chronic pancreatitis is associated with white race, female gender, patients on psychiatric medications, patients with depression and smokers.
