Post Visit: For Patients: Patient-specific education and local/national resource listings are provided in individual portal pages or to print and a virtual baby book is created for each young child. In addition, automated reminders to complete monitoring tools are sent for chronic conditions.
For Clinicians: Clinician support in the form of e-Chapters, MOC-4 credits, registries, reports supporting PCMH requirements, and condition-specific prepopulated templates to document evidence-based care are available.
