My name is Renae Boerneke and I am a clinical pharmacist practitioner in the UNC Infectious Disease Clinic. I attended pharmacy school at the University of the Pacific Thomas J. Long School of Health Sciences and completed a general PGY1 and specialty PGY2 in Infectious Diseases at the VA San Diego Medical Center. In my current role, I provide clinical oversight prior to discharge and throughout the OPAT course for all patients receiving OPAT through the UNC Infectious Diseases Clinic. Prior to discharge, I review referrals for OPAT patients by confirming appropriateness of the antimicrobial regimen, patient clinical status, and verification of appropriate laboratory monitoring which improve the transition from the inpatient to outpatient setting. Upon discharge, I provide clinical management and care coordination for OPAT patients within my collaborative practice agreement with infectious disease physicians. I interface with the various care teams (home health/home infusion) and assure laboratory tests are performed, evaluated and documented, monitor for adverse events and determines clinical course of action (i.e. urgent OPAT appointment, change in antimicrobial therapy, readmission), and assist with end of treatment care coordination.
